At the time of the incident two teachers were doing other activities such as cutting paper for the children's activities and another teacher was placing hooks outside the hallway for the children's backpacks and coats, instead of supervising the children.

On the operation?s video it was observed how a teacher, grabbed the child, pulled the arm, lifted, and made the child walk towards the back door.

The operation did not have a tracking system for each child coming in early in the morning.

The infant room was over ratio, the room had twelve (0-17 months) and nine (18 month- 2 years old), total of 21 children with one caregiver. The second classroom had children nine (2year olds) and five (3 year olds), total of 14 children with one teacher. The classroom was over ratio by three children.

A child in care was bitten by another child and the operation did not notify the parent(s) of this incident.

In the Toddler #3 classroom, there is a history of a child biting other children in care. This has been reported and acknowledged by the operation. The operation has not provided the level of supervision necessary to ensure each child's safety and well-being.

The operation did not have a current health inspection, the last inspection was done on 1/31/22.

A two-year-old child was in an empty classroom with no other children or staff. The child was not supervised for five hours until a teacher found him.

The operation failed to generate an incident/illness report for a child that was placed at risk.

Operation failed to notify Child Care Regulations in a timely manner of a child who was not supervised for five hours and was in an empty classroom.

A caregiver failed to escort a two-year-old child to the classroom and provide the child to the teacher. The child was unsupervised for five hours in an empty classroom until another caregiver found him.

Two infants, (12 month and 16 month) were combined with children as old as seven years old.

The operation does not have a current fire inspection, the last inspection was conducted on 4/6/21.

A child in care was left in a daycare van unattended for approximately 30 minutes.

The caregiver/driver was aware the child safety alarm inside the vehicle was not working; however, did not conduct a check of the van to make sure that no children were left behind.

The child safety alarm was not in working conditionsat the time of the incident in which a child was left unattended inside a vehicle for approximately 30 minutes.

A caregiver working for the operation left a child in care unattended in a vehicle after a transportation route. Child was left unattended inside the vehicle for approximately 30 minutes, placing the child at risk.

A staff member failed to account for all children in the van when unloading the children. As a result, a four-year-old child was left unsupervised in the van.
