Observed children files and 3 children files were missing health statements or did not check that the child had been seen within the last 12 months and they had 12 months to get the health statement to the daycare.

Observed multiple bottles with no names on them. I spoke with the caregivers about the child's names needing to be on the bottles.

There were 13 children ages 2 years old in a classroom with 2 caregivers. One caregiver took one child to the bathroom down the hallways and left the other caregiver in the classroom with 12 children which placed the classroom out of ratio by 1 child. This was corrected when the caregiver came back to the classroom.

One classroom said thqt they had 9 children but they only had 8 children in care.

Observed 4 staff files, the following trainings were missing from the files, Communicable diseases, Food allergies, Building and physical premises, Hazardous materials, administering medication, Age appropriate curriculum. Two caregivers were missing SIDS, shaken baby syndrom and early childhood development.

During my inspection, I observed a caregiver get up and turn an infant on their back. I asked the caregiver if the infant can roll over on their own and the caregiver said no., I asked how the infant got on their stomach and the caregiver apologized. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

Observed 2 infants sleeping on their stomachs, Licensing asked the caregiver if the infants rolled over on their own and the caregiver stated that they could not roll by themselves. I explained that the infants must be placed to sleep on their backs and not on their stomachs. The caregiver rolled both infants to their backs. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

Two teachers went inside of the room and did not realize that one of their children were still outside for about 1 1/2 minutes.

Observed a changing mat with multiple rips, this was corrected when the mat was removed from the changing table.

Observed a bottle of tylenol in the reach of the children in the classroom. This was corrected when the bucket with the medicine was put up higher.

Observed a concrete pole laying on the ground. Also observed the under pinning of the portable building exposed leaving access to under the building. Observed multiple ant hills on two playgrounds.

Observed 7 staff files and 3 were missing affidavits. The affidavit form was emailed to the operation.

Observed 7 employee files out of 24. Three employees did not have first aid or CPR certificates to view.

Observed the emergency practices form and the fire extinguisher has not been inspected monthly.

Observed 10 children files and 7 children were missing health statements.

During a DPFS investigation, it was found that the operation staff was notified of possible abuse of a child in care and the parent of the child was not notified immediately.

During a DFPS investigation, it was found that the operation staff was notified of possible abuse against a child in care and the operation did not report to licensing.

The playground had large cracks that could injure a child if their foot went into it. There was no mulch or soft cushion under the spinning top.

The music room had a set of outlets that were not covered. Caregiver covered outlets when requested.

An administrative penalty will be recommended as a result of this citation, per HRC ?42.078. A 3-month-old infant was in the crib asleep on his back with a bib on and a pacifier hooked to it.

There were outlets in the pre-k room that did not have covers. This was corrected during the inspection when outlet covers were placed in the outlets.

Several parents and children (x7) reported to CCR and stated that there was yelling, grabbing, and smacking children's hands occurring by caregivers at the operation.

Parents and children (x5) stated in interviews that children were called teased or were ignored by caregivers when bullying by other children occurred.

Of 10 children's records reviewed, 5 were missing authorization for emergency medical care.

Of 10 children's records reviewed, 5 were missing required written health statements.

Out of 10 personnel records reviewed, 3 staff members did not have current certification in First Aid and CPR.

Out of 10 personnel records reviewed, all 10 were missing notarized affidavits, 7 were missing education qualifications, and 4 were missing photo IDs.

While caregivers had required training in child development, caregivers were missing training in Emergency Preparedness, Communicable Disease, Food Allergies, Building Safety, and Hazardous Materials.

During a DFPS investigation, it was found toddler caregivers are only changing diapers twice per day.

During a DFPS investigation, cleaning supplies were observed accessible to children in the toddler classroom.

During a DFPS investigation, it was found a child sustained a minor injury requiring first aid and parents were not notified.

During a DFPS investigation, outlets were observed uncovered in the toddler classroom.

In three classrooms, caregivers were washing cups and bottles in the same sink that children and caregivers used to wash their hands. This was corrected at inspection when the director had the caregivers send the bottles and cups to the kitchen for cleaning.

There was an outlet without a safety cover in the four-year-old room. This was corrected at inspection when a caregiver placed a safety cover on the outlet.

Observed 3 employee files, the following items are missing from the files; Two caregivers who work with infant age children were missing trainings ins SIDS, Shaken Baby syndrom, early childhood brain development, abuse and neglect. One caregiver had an expired first aid/CPR card that expired 5/2023. The other caregiver did not have proof of first aid/CPR.

Observed 6 children records the following items were missing. All 6 files were missing health statements Two files were missing the emergency medical care information Two files did not have the operational policies recieved section checked off and Three files needed updated immunization records.

I observed a gate to the playground broken where it cannot be used to close the playground in. There was also a piece of wooden material that had fallen off the fence.

During an investigation it was discovered that the operation had not completed fire drills for multiple months.

Observed a volunteer in a classroom with children without a current cleared background check. This was corrected when the person left the operation and a new background check was put in.

Observed a caregiver in the classroom caring for children without a cleared background check. This was corrected when the caregiver left the operation.

Observed a caregiver in the classroom with children that had conditions to their background check that they could not be left alone with the children. This was corrected when another caregiver came into the classroom.

During an investigation it was discovered that a child was left on the operations transportation vehicle for around 2 hours.

During an investigation there was sufficient evidence to support a child neglected. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

During a walk-through of the outdoor active play space and equipment, children ages 3 - 4 years old were observed using equipment manufactured for children ages 5 - 12 years old. All of the children were removed from the equipment when addressed.

During a walk-through of the infant classroom, a dish, bowl of water and a dish towel was observed in the infant handwashing sink. The items were removed from the infant sink when addressed, bringing the facility into compliance.

There were no infant feeding instructions available for review for any infants not yet ready for table food. This was determined after a review of the infant subchapter.

There was no food allergy emergency plan available for review for a child enrolled in care that has been medically diagnosed with a food allergy.

During an inspection, two infants were observed using high chairs and the safety straps were not fastened. The safety straps were immediately fastened by a caregiver when addressed.

During an inspection, a combined group of children were observed in a classroom and the oldest child was more than 18 months older than the youngest infant in the group.

During an inspection, two classrooms were observed to be out of ratio. A three-year-old group was out of ratio by one child and a two-year-old group was out of ratio by two children.

During the investigation it was found that a caregiver used ridiculing language towards a child in their care .

There were no infant feeding instructions available for review for infants not yet ready for table food.

There was no annual training available for review regarding the following required content: administering medication, handling, storing, and disposing of hazardous materials, nor preventing and responding to a food or an allergic reaction.

Based on information obtained, the facility failed to notify a parent immediately regarding a situation that placed their child at risk while in care. This standard was evaluated during an inspection, bringing the facility into compliance. The facility notified the parent when addressed.

Based on information obtained, it was found that a child was left unsupervised in a classroom.
