The operation does not have documentation for having completed a fire drill for the month of February 2025. Deficiency was corrected at inspection after a fire drill was successfully conducted and documented to make the operation be up to date and come into compliance.

The operation does not have loose fill surfacing material or unitary surfacing material in the use zones under and around a medium sized jungle gym with a slide. The same applies to the use zone under and around an airplane play structure. Photos taken.

The operation failed to complete (the number of employees that Left employment in 2024) reporting requirement (survey) by the required deadline of January 15, 2025.

Director and one employee who transports children did not have current transportation training. Their last trainings were taken on 3/28/23 and 2/14/23 respectively.

One caregiver's file was missing current training in pediatric first aid with rescue breathing. Training card expired on 3/2/22. There was no current training available for review.

During inspection, in the school age class, cubbies used by children were not labeled. This was corrected at inspection after staff labeled the cubbies with the children's names.

Director and one caregiver did not have available proof of having current pediatric CPR training.

The operation has an expired gas leak inspection. The last gas leak inspection was conducted on 10/30/19

During inspection, a person acting as a caregiver was observed to be present and supervising children without cleared background checks, to include previously inactive canceled fingerprints. Therefore, the person was allowed to be present before receiving notification from the criminal background check unit that the person was eligible. This was corrected at inspection after the person/caregiver was removed from the operation (left the operation). The operation was informed the person cannot be present until eligibility has been determined.

A child's file was missing a health statement.

A school age child's admission information file was missing the name and telephone number of the school the child attends. This was corrected at inspection after assistant director added the missing information to the child's file after contacting the parent.
