During the monitoring visit, there were seven children who did not have up-to-date shot records for review during the monitoring visit.

The operation did not have a current annual sanitation inspection form available for review. The last inspection was completed in 2/2024.

During the monitoring visit, there were five children who did not complete the health statement section on the admission form, and the yearly documentation was not available as well for some.

During the monitoring visit there was one infant who did not have an up-to-date feeding form available for review. The last form available for review was completed in April.

Two out of ten children did not have a current shot record available for review during the monitoring visit.

During the monitoring visit, the fence on the playground was lifted up at the bottom of the fence. There was also trash on the playground.

The daily schedule posted in the classroom did not offer a second outdoor time for children while in care.

Five out of five staff did not have all the annual training hours available for review. The staff did not have trainings for child maltreatment/abuse, shaken baby syndrome, prevention of sids, understanding early childhood brain, emergency preparedness, preventing the spread of communicable diseases, food/allergies, understanding building safety, and handling and storing hazardous materials.

During the monitoring visit, there were two toilets in the boys restroom that did not have a seat, and the top part of the toilet was damaged in the classroom. The girls restroom had one toilet that was damaged on top.

Two infants in the classroom did not have written feeding plans available to review during the monitoring visit.

The director did not have the transportation training certificate available for review during the monitoring visit.

Five out of five staff completed CPR/First Aid training online.
