A review of the Employees Who Left Employment in 2025 data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

A caregiver was observed supervising a group of 13 chidlren with the youngest child in the group being 12 months and the oldest child being 3 years old. This was corrected at inspection when another caregiver stepped in, taking over the group of 4 infants and bringing the operation into compliance.

It was determined through video surveillance that a child in care left the building unsupervised.

A caregiver in the infant room that has a background check submitted as a volunteer was supervising 3 infants alone. The caregiver does not have educational documentation and does not meet caregiver qualifications. This was corrected at inspection when a qualified caregiver entered the classroom.

A child allergic to tree nuts did not have an allergy plan signed by the parent. The plan was only signed by the physician.

Upon reviewing of files it was determined that the director was lacking an up to date first aid and CPR certification.

Several areas of the chain link fence on the playground were detached from the frame causing possible entrapment hazards.

The operation did not have a playground safety checklist available for review. This was corrected at the inspection when the director printed a new form and conducted a maintenance check during the inspection.

The operation did not have a current gas inspection on file. The Directorcalled the plumbers immediately to schedule a gas inspection. The director will email the gas inspection once it is completed.
