One staff did not have a current CPR/First-aid training/certification available for review. The most recent certification was dated 03/15/2022-2024.

Four children files observed did not have a health statement available for Licensing staff to review.

The fence on the outdoor area was broken, in which the neighbor backyard area was exposed. The fence was contained broken boards that was on the ground and bricks near the broken pieces.

The childcare home files observed were not updated within the required timeframe. The childcare home did not update the children files to portray the updated required information by 09/01/2023. The files observed did not include an updated special care needs section, modifications, and/or the child's swimming ability.

The primary caregiver did not have training hours available for review. Based on the information obtained the primary caregiver has not obtained annual training since the previous training year, 2022.

The most recent CPR/First-aid certification for the primary caregiver was dated 04/22/2021.

The CPSC form was not available for review during the annual inspection. The primary caregiver acknowledged that they do receive the emails related to recall items. An email was sent to provider including the 2885 Form.

One infant served in care did not have an immunization record available for review. There was no documentation observed related to the childcare health care needs.

One out two staff files observed did not include a start/employment date for the staff. Based on the information obtained, the start date for the caregiver counted in ratio is 02/21/2024.

One out of three staff files were not available for review. Inspector was able to review the CPR/First-aid certification, but no personnel information was observed.

The operational policies do not contain policies on the promotion of active play and an emergency preparedness plan.

Five out five children records observed, whom have been enrolled for a year did not have a health statement available for review from their physician.

The primary caregiver personnel training file was not available for review during the monitor inspection. Inspector was not able to review training topics, and/or hours.

One caregiver observed caring for children file was missing the following training in orientation, which included: Administering medication, preventing and responding to emergencies due to allergic reactions, understanding building and physical premises safety, and handling, storing, and disposing of hazardous materials.

The primary caregiver CPR/First-Aid certification expired 3/19/2021. CPR/First-aid certification is scheduled for 04/24/2021. A caregiver within the home CPR/First-aid certification expired 03/19/2021.
