The crib sheets in the infant classroom were loose and provided an entanglement hazard. The cribs that are not currently being utilized by children were filled with toys and used as storage.

While reviewing the parent boad there was no documentation of safety emergency drills available.

During a walk through charger cords were accessible to children in care.

During a walk through loose fill was not maintained at a depth of at least six inches under stationary equipment.

Documentation of the playground maintenance check sheet was not available. *This was corrected at inspection when the check sheet was completed.

The safety lock on the cabinet in the infant room was not working and cleaning supplies was accessible to the children. *This was corrected at inspection when the cleaning supplies was moved.

Out of six staff files reviewed six were missing annual training in abuse and negelct.

Updated written feeding instructions for the infants was not available.

The annual fire inspection was not available.

The play yards in the infant room had mattresses that was not designed by the manufacture for the specific play yard. * This was corrected at inspection when the mattresses were removed from the pack n plays and removed from the operation.
