It was found during the inspection that the operation did not have a current Fire Inspection. The last Fire Inspection conducted was on 3/5/24.

It was found that a restrictive device was used for time out for a child.

It was found during the investigation that a child was not supervised at all times as required.

The Fire Extinguisher used in the van had not been serviced since 2017. This was corrected during the inspection by putting another fire extinguisher in the van.

It was found during the inspection that the outside material on the nap mats being used were cracked and missing in some areas and the cushion material inside the mats were exposed therefore they were no longer waterproof. This was corrected during the inspection by taping the areas needed in order for them to be waterproof.

1 out of 10 records reviewed was missing a Health Statement.

1 out 9 records reviewed was missing 1 hour of Abuse/Neglect Training. 1 out of 9 records reviewed was missing 22 annual training hours out of 24 required annual trainings.

It was found during the investigation that the operation did not make a report to Licensing when they became aware that a caregiver inappropriately disciplined a child.

It was found during the investigation that the Director of the operation was not ensuring the operation maintain compliance with Minimum Standards by allowing classrooms to be out of ratio, not supervising employees appropriately, not ensuring the employees are maintaining compliance with Minimum Standards, and providing falsified documents to Licensing.

Evidence obtained verified that the operation had 13 or more children in care and was placing infants with children that were more than 18 months older than the youngest child.

Evidence obtained supports that the Department was provided with false documentation of First Aid/CPR training certificates.

Evidence obtained during the investigation found that an employee inappropriately disciplined a child by spanking the child.

The operation did not report a change in Directors within the required timeframe.

It was found during the inspection that the operation does not have a qualified Director in place and hasn't had a qualified Director in place since 1/26/2023.

It was found during the inspection that a classroom was out of ratio. This was corrected during the inspection by calling in another caregiver to assist in this classroom.

The operation did not have an active background check on a caregiver and allowed the caregiver to be present in the Infant room and have access to children in care. This was corrected during the inspection as the caregiver was sent home during the inspection.

It was found during the investigation that a child was injured due to slipping on a wet floor.

Based on the evidence obtained, it was found that a 2-year-old child was left alone unsupervised on the playground.

The operation allowed a caregiver to be present in the Toddler room and have access to children in care before receiving their eligibility letter from the Criminal Background Check Unit.

Evidence obtained during the investigation found that an infant was seated at the table with no activities provided to the infant for over 30 minutes.

Evidence obtained during this investigation found that caregivers were not supervising children appropriately.

Evidence obtained during the investigation found that two infants were allowed to sleep in a bouncer for whole duration of their nap.

2 out 10 records reviewed was missing school information.

2 out of 10 records reviewed didn't have current immunizations in their file.

It was found during the inspection that two infants were sleeping in cribs without a crib sheet on the mattress pads. This was corrected at the inspection.

During the investigation, it was determined that the staff was aware of a child receiving medical attention after sustaining an injury while in care but did not report to Child Care Regulations within two days of becoming notified.

During the investigation, it was determined that the room was not set up for the caregiver to see all infants at a glance or be able to intervene in a timely manner resulting in a child being injured and needing medical attention.

It was found during the investigation that the operation released children to an unauthorized individual.

The Merry go round and the center spring seesaw on the Pre-K playground did not have at least 7 feet of cleared space around the equipment.

The operation allowed a caregiver to be present in the Infant room and have access to children in care before receiving their eligiblity letter from the Criminal Background Check Unit.

During the inspection it was found that playground equipment was placed in front of the exit of a slide only given 3 feet of cleared space. This was corrected at the inspection by removing the piece of equipment in front of the slide giving the proper amount of cleared space from the exit of the slide.
