During the visit, there was an 11-month-old infant who was not strapped in the high chair. The caregiver put the straps on the 11-month-old infant upon request in the high chair.

During the monitor visit, there were two employees who did not have the first aid/CPR certificate available for review during the monitoring visit.

During the monitoring visit, the school-age classroom was in the movie room watching a movie during naptime/rest time. The caregiver removed the children to the school-age classroom during the monitoring visit to the classroom to rest on their mats. The television was turned off in the movie room.

During the monitoring visit, there were two infants without feeding forms signed by the parent last dated in September and November of 2024.

During the monitoring visit, there was one caregiver with an inactive background check. The caregiver was in the classroom supervising young children during naptime. The director submitted a background check while present, and the caregiver left the operation during the monitoring visit.

During the monitoring visit, two children did not have the food allergy emergency plan form available for review that is signed by the doctor and the parent.

The operation had a fire inspection on 12/1/2023. The operation completed the repairs, but the fire department has not returned to view the repairs to complete the annual fire inspection.

One classroom cabinet needs repair. Another cabinet in a classroom was missing two knobs. The outdoor grounds need mulch added to the surfaces. The barrier surrounding the large playground equipment needs repair. The slide area on the playground equipment mulch was low. The bathroom had an opening in the wall that needed covering.

Four out of ten children did not have up-to-date shot records.

Two children did not have an allergy form signed by the parent and the physician.

The caregiver did not assist the toddler with washing their hands after diapering.

The operation did not provide fire drills consistently up until July. The operation did not complete shelter or lock down drills.

The operation did not have a carbon monoxide detection system.

Four out of ten children did not have the health statement section complete.

Based on the investigation, a caregiver did not use good judgement by verifying if a child needed to be transported from school to the daycare.

One of the bathrooms was cluttered with many objects obstructing the access for children to use the restroom.The second bathroom had a urine like smell with a rug rolled up in a corner and plastic tubs in the bathroom. The floor was also filled with dirt like substances and the tissue on the floor.

The sinks that the caregivers use for diaper changing are also being used for washing bottles.

Three out of four employee files were missing updated annual training records.

Both of the vehicles that are being used for transportation do not have a child safety alarm installed.

The operation did not document recent fire drill.

While present at inspection child care regulation inspector heard a caregiver yelling at children for discipline. This was corrected at inspection when child care regulation inspector brought this to the attention of the director and the director went into the classroom and redirected the caregiver. No further yelling was overheard during inspection.

A caregiver had 18 out of 24 training hours avaliable for review.

The operation did not have the Licensing Parent Notification Poster posted and could not locate it during this inspection. Operation printed this item during inspection.

A childs file did not have proof that the parents received a copy of the operational policies for the operation.
