The operation completed 5 of the 8 required emergency drills for sheltar/severe weather and lockdowns.

Enrollment agreements were incomplete for parent's rights, signatures and water safety permissions.

Cleaning supplies were observed underneath the hand washing sink in a classroom. This was corrected during the inspection when the supplies were removed and placed out of reach.

Components of the operational policies were incomplete. Parents rights, infant safe sleep.

A child was observed walking unsupervised to the bathroom.

Three children were observed to be sitting in a restrictive device while the other children were playing around the room.

Two children had bottles of milk in the crib. One child was drinking milk while sleeping, a second child was observed laying down with a bottle.

Four employees had not received the required 8 hours of pre-service training prior be being counted in ratio. Two caregivers were working with children, neither had SIDS/Shaken Baby, and CPR/FA.

An employee did not have the required completed records for employment. Missing from the file were education, pre-service training, CPR and F/A, photo ID, notarized and preapplication affidavits. Three other employees were observed working in the capacity of caregivers and did not have an employee file. No files had been created for these employees.

A child was observed walking from the bathroom back to his classroom unsupervised.

Two children were observed sleeping in restrictive devices.

A caregiver with a pending background was present at the operation and in a classroom caring for children. The caregiver was removed from the classroom and placed in the training room for training.

Four employees did not have the required completed records for employment. Missing items included: pre-service training, CPR and FA, SIDS/Shaken Baby, notarized and preapplication affidavits, photo ID's, and proof of education.

The operation had not received the background determination for a caregiver before giving the employee access to the children. A caregiver with a pending background was counted in ratio and had access to the children. Two employees were observed counted in ratio with access to children and did not have submitted background checks. The caregiver was removed and sent home.

The operation was observed not following the conditions of the CBCU. A caregiver with a provisional background was observed alone with a group of children. The caregiver was placed in another room with a caregiver without conditions.

Four employees had not received the required 8 hours of pre-service training prior be being counted in ratio.

During the inspection, a caregiver did not wash hands after diapering a child.

A caregiver was observed watching two classrooms. The caregiver was standing in the hallway at the door of the classes. This was corrected when the caregiver returned and entered the room.

Nap mats did not have enough space for walking between each one.

The parent board was missing the most recent licensing inspection, activity plans for each age group of children, menus, the local licensing number and address and the child abuse and neglect hot line.

The operation did not have documentation of the current fire inspection.

The director and one other driver of the van did not have current transportation training.

During naptime, there were no additional employees in the building to answer the door or help caregivers who were supervising the napping groups. This was corrected during the inspection when the director and other caregivers returned to the operation.

Children under two years old were watching television. This was corrected during the inspection when the television was turned off.

During naptime, two rooms were observed without adequate lighting.

During the investigation inspection, sufficient evidence was found to determine that proper supervision was not being maintained when two caregivers stepped out of their classrooms, leaving the children inside unsupervised. This was corrected during the inspection when the caregivers returned to their classrooms.

During the inspection, a caregiver was observed using a changing station to change two children. The diaper surface was covered with a multi-colored non-absorbent material.

A caregiver was observed not washing the hands of a child. The caregiver carried the child back to the play yard without washing the child's hands.

During the inspection one caregiver were observed leaving the classroom to answer a call from the director, leaving the children unsupervised in the classroom with the inspector.

Children were observed watching a non-activity related cartoon program on the television while eating snack.

A caregiver was observed leaving the room to go to a hallway bathroom to wash the hands of a child, leaving 5 children in the room with one caregiver. The ratio of the room is 4 to 1. The caregiver returned to the room, bringing the room into compliance.

During the inspection a caregiver was observed caring for children and was not in ratio. There were 6 children observed in care, bringing the room out of ratio by 2 children. The ratio is 1 caregiver to 4 children.

During the inspection, play yards were observed with blankets, toys and bumpy padding and hats. This was corrected at inspection when the items were removed by the caregiver.

During the inspection, a child was observed sitting in a restrictive device during the rest period. This was corrected when the caregiver was removed the child and placed her at a table with an activity.

Children were observed in two classrooms watching screen during the rest period.

During the inspection, 6 play yards were observed with mattresses that did not fit snugly the sides.

Children were observed during a designated rest period, with little to no lighting and the doors were closed.

During the inspection an employee was observed supervising two classrooms of children from the doorway of the other classroom.

A child was allowed to be in care at the operation without any of the required for a completed therefore not having any emergency contact information for the child.

Children were observed napping near the entrance/exit of two classrooms preventing the door from being opened all the way. One child was also observed napping underneath a table.

There was not an electronic safety alarm installed in the transportation van.

Two infants were observed swaddled in the infant classroom for naptime.

Two children's records of six that were read did not have emergency medical authorization.

There were two caregivers present without cleared background checks.

During the inspection it was found that 2 out of the 6 children records reviewed did not have admission requirements completed and an up to date immunization record.

During the inspection it was found that the two year old classroom and infant classroom had televisions playing. This was corrected at inspection when the television in both classrooms were turned off.

During the inspection it was found that the diaper changing mat in the bathroom had two small tears. This was corrected at inspection when a nap mat was used as the diaper changing mat.

During the inspection it was found that the operation had a whole in the wall in the gym area that was accessible to children. It was also found that the operation outdoor play structure had cracks and a large whole with rough edges near the slide, and the foamed mats used for the outdoor flooring had large gaps.
