The garage outside and slide stairs have sharp edges that are accessible to children.

There was a 4 month old sleeping in a crib, on a boppy pillow, covered by a blanket.

The shelf in the pre-k classroom is not secured to the floor or wall, creating a tipping hazard.

There was a caregiver, who had worked at the operation since Sept 2022, who's CPR and first aid training expired on 06/30/2024.

A transportation vehicle that was purchased in 2016, that seats 8, did not have a child safety alarm installed.

A caregiver did not have documentation of annual training for the previous year.

Two caregivers did not have documentation of orientation on file for review.

There was 1 person that had an expired CPR Certification. The certification expired on 8/13/2022.

Three infant feeding instructions have not been updated since August 2022.

The operation did not have the following postings posted: *Current Employee List *Parent Notification Poster *Keeping Children Safe Poster The Director posted the required postings during the inspection.

During the follow-up inspection the televisions were re-cited when they were not anchored to prevent tipping. They were all removed during the inspection and cannot be present back at the operation until they are mounted or anchored.

During the follow-up inspection it was found that the daily grounds safety checklist was still not being completed in full each day. This was recited and a blank form was emailed to operation to use moving forward.

During the monitoring inspection it was observed that there were 4 TVs at the operation sitting on shelves, none of which were anchored or secured to the shelf they were sitting on.

During the monitoring inspection it was observed that the playground slides did not have 6 inches of loose fill around the play structure or at the bottom of the slide.

During the monitoring inspection it was observed that the 5 infants in the infant room only had 2 small toys accessible for their use.

During the monitoring inspection it was observed that there were cleaning supplies left out on low counters, accessible to children.

During the monitoring inspection it was observed that the caregivers were not following the daily scheduled outdoor time, as the children were not having two oppurtunities daily to play outside, even when the weather permitted. During the inspection the weather was 75 degrees with no chance of rain in the forecast and none of the children played outdoors during their scheduled time.

During the monitoring inspection it was observed that a child used the bathroom and did not wash their hands afterwards.

During the monitoring inspection infants were observed to be seated in high chairs without the straps buckled and fastened around the child.

During the monitoring inspection it was found that the daily grounds safety checklist had not been completed since May 2021.
